As a concierge care practice, we do not accept or bill insurance for our services.
Insurance coverage for hormone replacement therapy, weight management, and personalized wellness care can vary significantly between plans, and insurance requirements can limit the services, treatment options, or time we are able to provide.
We gladly accept HSA and FSA payments. Upon request, we can provide a superbill with applicable diagnosis codes that you may submit to your insurance company for possible out-of-network reimbursement. Because coverage and benefits vary by plan, we’re unable to guarantee reimbursement and recommend confirming your benefits directly with your insurance provider.
Initial consultations are a one-time $150 fee, and there are no additional consultation fees for future appointments. If you enroll in a Weight Management program, the $150 consultation fee will be applied toward your first month of the program.
Our concierge model is designed to make ongoing care simple and predictable. All monthly memberships and programs include ongoing communication with your provider during business hours and all follow-up visits. Weight Management programs also include your weight management medication, and HRT memberships also include prescribed hormone medications and all follow-up laboratory testing.
